Below you will find all the plan forms and applications you‘ll need. The forms and applications are available as downloadable and printable PDFs (portable document format) files. Enrollment Forms
 

Enrollment is easy. Simply complete one of these enrollment forms and return it and the beneficiary form to your employer. Remember to save a copy for yourself. Here you‘ll find a percentage election enrollment form, a dollar election enrollment form and an for hourly election form. Check with your employer or union to determine which enrollment form is appropriate for your plan.

Rollover Form
 

This form is used to roll money from another retirement plan or IRA into your Supplemental Income 401(k) Account. The form outlines exactly what types of money can be rolled into the plan and tells us how you would like the money invested in the plan.

This form must accompany your enrollment form and determines who will receive your benefits in the event of your death.

Change in Elective Contribution Amount Forms
 

Interested in changing the amount you are investing in the Plan? Simply complete one of these forms and return it to your employer. Remember to save a copy for yourself. Below you will find a percentage change in election contribution amount form, a dollar change in election contribution amount form, and an hourly change in election contribution amount form. Check with your employer or union if you are unsure which form is appropriate for you.

Loan Form
 

Need to initiate a loan from your plan account? Just print out this form, read and fill it out completely and return the form to the address on the form. Remember to save a copy for yourself and be sure to read and sign the loan letter that goes with the form. Call the participant service line at 800-560-3243 for assistance in completing the loan application.

NOTE: Effective immediately the minimum loan amount is $2,500. Additionally, consolidated loans must be repaid by the maturity date of the original loan. You may not qualify for a loan unless you are working for a participating employer.
